<p><strong>Mechanism of action</strong></p>
<p>The parietal cells of the stomach secrete H+ with the help of H+K+ ATPase enzyme present in the plasma membrane and this is the final step in gastric acid secretion due to all stimuli. Proton pump inhibitors accumulate in the parietal cells where they specifically and irreversibly inhibit H+K+ ATPase and thereby inhibit gastric secretion. Omeprazole gets activated in the acidic environment of the stomach to sulfenamide which binds covalentely with H+K+ ATPase. The binding is irreversible. A single dose almost inhibit gastric secretion.</p>
<p><strong>Adverse effects</strong></p>
<p>Omeprazole is well-tolerated. Prolonged acid suppresion may allow bacterial over growth in the stomach. Dizzinesss, headache, diarrhoea, abdominal pain, nausea, arthralgia and rashes may occur. Long term administration result in vitamin B12 defficiency due to its absorbed absorption, decreased rastrin levels, atrophic changes in the stomach noticed after 3-4 years of use.</p>
<p><strong>Uses of PP Inhibitors</strong></p>
<p>Used in GERD that does not respond to H2 blockers. They also form a component in Helicobacter pylori treatment regimen. PP Inhibitors are also used in Zollinger Ellison Syndrome assosciated with gastrin secreting tumors.</p>
<p>Sodium Carbonate, an antacid is rapid but short-acting. it nueutralize the gastric acid and raise the pH of gastric contents.</p>
<p>GERD (Gastroesophageal Reflux Disease) - reflux of gastric acid contents into the oesophagus results in 'heart burn' dut to oesophagitis. Chronic oesophagitis result in changes in the oesophageal mucosa which could be a premalignant condition. Based on severity, GERD is treated with antacids ,H2 receptor blockers. In moderate to severe cases, proton pump inhibitors like omeprazole are the drugs of choice.</p>
<p>Zegerid is used in adults:</p>
